So you’re considering… Aesthetic Rhinoplasty, or a ‘nose job’

What is it?

A cosmetic procedure to improve or correct the appearance of the nose.

Why do people have it done?

To correct bumps or misshapen features of the nose, to reduce or increase its size or to change its shape.

The procedure:

Rhinoplasty involves surgery on the bones and cartilage of the nose. It is usually performed under general anaesthetic and takes from two to four hours to complete, depending on the complexity of the deformity of the nose. The procedure itself can be simple or quite complex. A patient might simply require the correction of a bump, which can be shaved down. The tip of the nose can be refined with the removal of some cartilage. Or a nose can be straightened or narrowed by the breaking and resetting of the nasal bones (osteotomies). Implants or cartilage grafts, using the patient’s own tissue, can be used to correct the structure, or combinations of the above may be used to achieve the best results.

It is important for anybody considering cosmetic surgery to be aware of what the procedure involves. This particular surgery can be done either as a day case or as an in-patient case whereby the patient is usually discharged the day after the surgery. Any packing inserted after the procedure (internal nasal splint) and the external nasal splint that helps maintain the shape of the nose after the procedure will usually be removed within a week to ten days. Patients should take time off work and avoid groups of people for a few weeks, to reduce the chance of catching a cold, which could result in infection. Swelling and bruising should have subsided by the third week.

Rhinoplasty is one of the surgeries I perform most frequently as lead consultant plastic surgeon at the Blackrock Clinic. I love it because it can have significant impact, both cosmetic and psychological, on the patient. When carried out by a skilled surgeon, a ‘nose job’ can have a dramatic effect on the face and can give or restore a sense of self-confidence to someone who has perhaps been unhappy with their appearance for a long time.

When it comes to projecting the desired results or what the patient hopes to achieve from the surgery, pre-op photographs are a very helpful tool. They are beneficial while patients and surgeon are discussing the issues of concern and, more importantly, the potential or expected results. Some 90%of the success of aesthetic rhinoplasty surgery is in making sure that the operating surgeon and the patient agree on the operative goals.

The use of modern advancements in 3D imagery mean that a patient can get a real idea of projected results during their consultation and I use the latest 3D imaging system Crisalix or Vectra 3D System. This will go a long way towards making sure both parties are on the same page, as it were, regarding what is achievable.

As such a skilled surgeon and a detailed consultation process are essential. The nose is the centre of the face. Rhinoplasty is a precision surgery requiring meticulous attention to detail and good artistic skills, not to mention the technical aspect of cosmetic surgical skills.

With this aim, during consultation the nose and face are closely examined. Angles and aesthetic lines of the nose and face are taken into account and the nose is further divided into smaller units known as the nasal sub-units. This all helps to ensure the best results and a uniformity or harmony of features when surgery has been completed.

As with all surgeries, results will vary from person to person, as they should. Your nose should work aesthetically within your own features, and a good surgeon and proper consultation process will help achieve this. It’s also important to manage expectations and remember the aim is improvement, not perfection.
